Form W-9
Description
A person who is required to file an information return with the IRS (such as a 1099-MISC or 1099-DIV) must get your correct social security number or employer identification number to report on the information return. For example, if you worked as an independent contractor for a business during the year, that business may send you a W-9 in order to obtain your social security number to put on your Form 1099-MISC.
Use Form W-9 to give your correct social security number or employer identification number to the person requesting it and, when applicable, to:
- Certify the social security number or employer identification number you are giving is correct (or you are waiting for a number to be issued).

- Certify you are not subject to backup withholding, or
-
- Claim exemption from backup withholding if you are an exempt payee.
